Tunstall Integrated Care Platform (ICP) Connected Health Solution

Supplied by Tunstall Healthcare

Tunstall's Integrated Care Platform (ICP) offers a way of delivering tailored care for patients with long-term conditions in their own homes, helping improve quality of life and prevent avoidable hospital admissions.

ICP myclinic involves the remote monitoring of a patient’s vital signs and health condition from their own home. ICP triage manager collects the patients’ vital sign and interview information from the telehealth hub for review on a regular basis. Abnormal readings for the patient are flagged for follow up and intervention such as clinical coaching, nursing visit or GP appointment.

The collected data may also be passed on to the patient’s doctor for further review or intervention. The ICP solution allows the patients’ care team including community nurses, GPs and specialists to closely monitor the patient’s condition and intervene before it reaches crisis level and results in hospitalisation. ICP also offers video conferencing at the press of a button, allowing patients to maintain face to face contact with their clinician regardless of their location or medical state. Tunstall's ICP Connected Health solution may be used for, but is not limited to:

  • Long term and chronic disease management
  • Case management
  • Early hospital discharge
  • Remote communities where access to doctors is limited

Tunstall Healthcare provides assistive technology solutions to support people with a wide variety of care needs to live safely and independently in their own home.

The central hub of Connected Care is Tunstall’s personal medical alarm that can be used to quickly and easily make contact with Tunstall’s 24 hour monitoring centre or interfaced with a range of sensor devices to monitor the home environment.

Connected Care can be active or passive: active Connected Care requires the client to activate an alarm call for assistance via their Tunstall medical alarm, such as pressing a pendant trigger; passive telecare automatically notifies the monitoring centre of abnormal environmental conditions, such as no movement or movement at unexpected times via a range of sensors.

Tunstall can create custom Connected Care packages to suit various needs, conditions and abilities.

A person’s clothes in many ways form part of an individual’s identity with older people often taking great pride in their appearance.

RFID tagging attaches a small durable button to clothing to allow easy laundry identification. Each button is programmed to store up to 200 characters of information to identify the owner.

This information can include the person’s name, unit, room number and any other information that they may wish to keep private e.g. skin related conditions or allergies to washing powders.

This allows all items of clothing can then be collected for washing together.
